Will be displayed in the inventory for others to recognize you by. Follow the order: [First name] [Surname].

*Email address
An activation email will be send to this address.

UT employee / student number
Used for matching the barcode when scanned from your UT card. Starts with 'M' or 'S' followed by 7 digits. Leave blank when not applicable.
